Proper procedures must be followed to assure the integrity of the specimen, its safe and expedient delivery to the laboratory and accurate processing once in the laboratory. The sample requisition form is the most important document to guarantee accurate specimen identification. Completing the requisition form properly, accurately and in its entirety is crucial. How to prevent common errors:
- Collect sufficient quantity.
- Be sure to use the correct container for specimen collection.
- Ensure proper and accurate labeling of specimen to provide all pertinent information.
- Use the tamper evident requisition form barcode sticker.
- Tighten specimen container lids, preventing leakage and/or contamination of specimen.
- Have the donor initial the tamper evident requisition form barcode sticker.
- Make sure the requisition form donor identification information is the same as the information on specimen container.
- Indicate the required test.
- Ensure collector signature on the requisition form.
- Enclose the requisition form with the specimen.

Request kits
For your convenience USDTL provides test collection kits at no extra charge. Orders are processed within 24 hours of receipt and shipped via FedEx Ground. Allow three to five business days to receive your collection kits depending on your location.
